For Enrollment Process:
- Proof of payment.
- Below Poverty Level or BPL card (in case the beneficiary belongs to the BPL category).
- Address Proof.
- Disability certificate issued from the District Hospital or appropriate Government authority (Self-attested).
- Income certificate (Self-attested) of the parent/ guardian competent authority as issued by the State (in case the beneficiary belongs to the APL category).

How to Apply Online for a Health Card in Niramaya
To obtain a Niramaya Health Card, an online application is required. The steps to enroll for a Niramaya e-card are as follows:
- Go to the nationaltrust.gov.in official website.
- Once there, select Niramaya from the “Scheme” section by clicking on the left-hand side. There, you will find all the facts about the scheme.
- Select “Download.
- Click the “Download” option after reviewing the contents of the Niramaya Health Insurance plan. The Raksha TPA Pvt Ltd. Centre List, the Enrollment Form, the Renewal Application Form, and the Claim Form are your four possibilities.
- Click “Download Enrollment Form”.
- Select the “Download Enrollment Form” option from the list of four. Have all the necessary information listed on the form close at hand.
- Submit the form
- Send the completed form to the closest RO. The rest will be handled by the RO, who will also complete the online Niramaya application form and upload the necessary scanned documents after the originals have been verified. After finishing, the RO will pay the application fees and submit the properly completed form on the National Trust portal.
